PHQ-9 Depression Screening: Questionnaire and Score Guide
Learn how the PHQ-9 describes depressive symptoms, what its 0–27 score means and why screening is not a diagnosis. Arabic questionnaire available.
Arabic interactive tool available for adults.
The tool opens on a separate page. Answers are not sent to a clinician.
What it measures
PHQ-9 asks about nine depressive symptoms during the past two weeks. It helps describe and monitor symptoms; assessment is still needed to understand their cause and clinical significance. The Arabic questionnaire is available for adults aged 18 and over.
Reading the score
The total is 0–27: 0–4 minimal, 5–9 mild, 10–14 moderate, 15–19 moderately severe and 20–27 severe symptoms. All nine responses are required. The optional impact question is not scored. A score is not a percentage likelihood or a treatment recommendation.
Safety and privacy
The item about death or self-harm requires attention independently of the total. The app does not estimate suicide risk. Seek prompt professional support for these thoughts and emergency care if there is immediate danger. The page is not monitored; answers remain in page memory and are not sent to a clinician.
